Three Firefighters Injured In Pawtucket Fire

Around 11:30 on Friday morning Pawtucket fire alarm dispatched Engine's 1,2,3 Ladder 1 & Battalion 2 to 111 Randall Street for a reported structure fire. Heavy black smoke vented from the side three windows and eves as the crew of ladder one finished the roof vent and the incident commander ordered them to evacuate the roof. In the process of vacating the roof one firefighter broke thru the roof and slid down the incline stopping inches from the edge, the second firefighter without hesitation attempted to make the rescue and also broke thru up to the waist. The interior fire companies were evacuated as heavy fire began to vent thru the roof in numerous locations and windows on side four. The two firefighters now trapped on the roof were able to safely make it to Ladder Two who had setup on side one across from Payne Park and were evaluated by rescue personnel once on the ground. A mutual aid box was transmitted thru Metro Control for station coverage and mutual aid crews were dispatched into the city to assist from surrounding municipalities. Heavy fire conditions in the building were brought under control with a exterior attack in about a half hour and crews reentered to chase smaller pockets of fire and overhaul. The cause of the incident is under investigation by fire prevention and the state fire marshals office. The three firefighters injured were transported to Memorial Hospital for further evaluation.

Shortly before 11:00 on Thursday night Providence Fire Alarm dispatched Engine's 7,2,9 Ladder's 4&7, Special Hazards, Rescue 5 & Battalion 3 to 262 Brown Street for multiple calls reporting a house fire. Engine Two arrived within minutes and reported a "Code Red" with fire showing from the side one exterior porch of a occupied 2.5 story wood frame and stretched a line while Ladder Four arrived and rescued a occupant trapped on the roof on side two. Engine 4, Ladder 8, Engine 5 with Air Supply & Division 1 responded on the working fire assignment. Fire extended up the side one/four wall due to balloon frame construction and took hold of the third floor and a special call was made for an additional one and one (Engine 14, Tower Ladder 1 & Rescue 3). Command advised Hidden Street as side one as the third floor lit up and the evacuation order was given. Heavy fire vented thru the roof and dormers as ladder pipes were setup from Ladder Seven, Ladder Four & Tower One. The incident command requested an additional two engine's & and a additional ladder to the scene as division one transmitted the second alarm, Engine 12, Engine 8 & Ladder 3 responded. A exterior operation made quick work of the fire having it under control in about an hour of the dispatch. The cause is under investigation however talk on scene suggest a possible arson. Several minor injuries were reported, the providence canteen responded to provide rehab on what was a very cold New England night. Providence Public Works also responded with Sanders as water runoff flowed down the hill towards Onley Street and froze.

Worcester Firefighter Killed In Three Alarm Fire

Shortly after 04:15 on the morning of Thursday December 8th Worcester EMS reported a building fire behind the old Providence Street Station adjacent to the previous fire building on Arlington Street. The Worcester Fire Alarm Office transmitted Box 12-07 Arlington Street at Dorchester Street for the vicinity of 44 Arlington Street, Engine's 12,6,13,2 Ladder's 1 & 3, Car 4, Rescue 1 & Ladder 7 (RIT) responded. Car 4 reported fire showing from Madison Street and confirmed the working fire moments later engine twelve arrived at 49 Arlington Street a typical Worcester style three decker with heavy fire showing from the B & C sides and reported possible people trapped on floor two. Car four arrived and transmitted a second alarm with Engine's 7,15 Ladder 5 & Car 3 responding. Interior crews were forced to back out of the upper floors due to heavy fire conditions while exterior crews began to protect numerous exposures. Ladder three reported an exterior porch collapse in the rear and all interior fire crews were evacuated from the building and fireground operations went defensive. A One story garage became involved to the rear and due to a live electrical wire down command special called Engine 4 to respond off Providence Street and handle that fire as police began evacuating the surrounding homes in the affected area. A large majority of fire was darkened down with the exterior attack and firefighters from Rescue One were allowed to reenter the building to preform a secondary search for the reported missing occupant. As members began to search the second floor the safety officer broke thru with urgent traffic reported a full collapse of the rear of the building seconds later engine thirteen advised a full wall collapse into the "C" exposure and reported hearing a PASS device sounding. A third alarm was subsequently sounded bringing Engine's 16,3 & Ladder 2 followed shortly thereafter for an additional special call for the Special Operations (Engine 5) & Ladder 4 while on scene firefighters began to search the pile for the two missing men. Firefighter Jon Davies was extricated and transported to UMASS Memorial Hospital where he died of his injuries, Firefighter Brian Carroll was extricated an hour later from the basement and transported to UMASS Memorial Hospital where he was treated for serious but non life threatening injuries.

The incident occurred five days after the twelfth anniversary of the Worcester Cold Storage Warehouse Fire that killed six firefighters on December 3, 1999. The last Worcester Firefighter killed from injuries received while on duty was Lieutenant Donald Quinlan who was injured while operating at a New Years Eve fire at 634 Grafton Street, Box 07-14 on December 31, 2007. Lieutenant Quinlan was a 30 Tear Veteran of the Department and was assigned to Rescue 1, He also previously served as the Chief of the Barre, Massachusetts Fire Department he succumbed to his injuries on March 22, 2008.

On Thursday, December 15, 2011 thousands of firefighters from all over the United States and several other countries gathered together for Firefighter Davies Funeral which was held at The Our Lady of Mount Carmel Church On Mulberry Street. Mutual Aid from Grafton, Shrewsbury, Milford, Marlborough, Ashland and Framingham responded to city calls so all Worcester Firefighter could attend the services. Pipes & Drum Bands from across New England marched the fallen firefighter to his final resting place as the rain began to fall. A large gathing of firefighters gathered at the DCU Center later that afternoon. The man who was reported to be still inside the building that trapped the two firefighters was found safe the day of the wake and was arrested on a previous case not involving the fire.

There have been many fires in the Arlington Street area recently On November 24, 2010 a electrical fire was extinguished at a home at 56 Dorchester Street, September 18, 2011 around 4am a fire was reported at 48 Arlington Street, & October 3, 2011 around 4am 51 Arlington Street firefighters extinguished a porch fire. all of which occured since the 109 Year Old Providence Street Station was closed in late 2008 the fire Department relocated Engine 12 to the newly opened Franklin Street Station & Ladder 5 was moved to the McKeon Road firehouse, UMASS Worcester EMS now occupies the station known locally as "The Rock." Additionally in 2005 a Arlington Street building was destroyed by a early morning multiple alarm fire.

Once the fence was removed firefighters made quick work of the vehicle fire using one line there was no extension to any structures. The fire is under investigation by the fire prevention office.

Around 11:30 on Sunday night the Hopkington public safety office received a call from a worker reporting smoke in the Thames River Tube Mill located at 64 High Street (Route 216) in the Ashaway District. The Ashaway fire chief arrived and reported a light smoke condition from the two story wooden 100x300 mill building as volunteers responded to the station located less than a mile away at Route 3 & Route 216 for the apparatus. Engine 414 arrived and began an interior attack on a fire believed to be in the office area but were evacuated shortly thereafter as heavy fire conditions towards the rear of the building lit up the night sky. Due to the rural area and no hydrants in the district four alarms and several tanker task forces were requested in quick succession through Southern Fire Control. Fire took hold of the entire wooden portion of the building sending brands towards numerous exposures and additional special calls were made. Eighteen to twenty tankers were dispatched from the surrounding Rhode Island & Connecticut area some from over forty-five minutes away while two water holes were established to fill the incoming tankers. Members drafted from the Ashaway River at Laurel Street & High Street and used the water tank at the Hopkington Industrial Park additionally firefighters also drafted from the river in the rear of the fire building to maintain a water supply for suppression efforts. At 00:26 on Tuesday Morning Pawtucket fire alarm dispatched Engine's 1,2,5 Ladder 1 & Battalion 3 to 440 Pawtucket Avenue between Waltham & Sterling Streets for a reported structure fire. Numerous calls flooded into the state 9-1-1 office as the police arrived and reported the front of the building engulfed, seconds later engine one arrived and reported a "Code Red" on channel one. Lieutenant Masse the officer in charge of engine two arrived and transmitted a second alarm on channel two reporting the exterior front porches of a two story occupied woodframe dwelling fully involved shortly after the broadcast the porches collapsed. Engine 3 Ladder 2 & Rescue 1 filled in on the code red assignment while ladder one who is utilizing the reserve ladder three setup to vent the roof. Heavy fire conditions on the exterior extended into the first and second floors on side one while firefighters began an aggressive interior attack to handle the extension and preform primary and secondary searches. Exterior crews worked on the collapsed porches and exposure protection on sides two and four. The majority of the overlapping fire was knocked down in about a half hour leaving an extensive overhaul operation, two parked vehicles, a telephone pole and a fence also suffered heat related damage the building of origin 442 Pawtucket Avenue was heavily damaged. No injuries were reported during the fire and the cause is currently under investigation by the fire prevention office. Engine One arrived and reported a "code red" subsequently Engine 3, Ladder 2 & Rescue 1 filled in on the working fire assignment. Captain Thurber the officer in charge of Engine Two reported a possible occupant trapped on the top floor and assisted engine one who was already engaged in an aggressive interior attack. Battalion Chief McLaughlin arrived and established incident command followed shortly thereafter by the transmission of the third alarm which brought Engine's 4,6 & Rescue 2 as fire broke thru the roof towards the rear of the three story multiple dwelling. Ladder One who is operating with reserve ladder three setup towards Randall Street and Ladder Two under control of Lieutenant Slater accessed the fireground via Pawtucket Avenue and setup. Heavy smoke conditions turned to venting fire upon completion of roof operations and crews evacuated the roof, on the interior crews pulled the ceilings exposing the heavily involved attic and began to get water on the fire using two lines. Companies began to make progress having the overlapping fire knocked down in about twenty minutes, with all city apparatus at the scene a mutual aid box was struck for station coverage. The primary and secondary searches were negative however several occupants and two firefighters were evaluated on scene for possible smoke inhalation. The home suffered extensive damage to the upper floors and the cause is under investigation by the fire prevention office.

Massachusetts State Police arrived and confirmed a patient trapped in a overturned vehicle. Car two arrived and requested two tow trucks to the scene to assist in the extrication as firefighters began to stabilize the vehicle which was hanging over the guard rail. Command special called Engine One with a second set of jaws and North Attleborough Engine Five for additional cribbing, due to the prolonged extrication Medflight was requested however denied due to weather conditions. Over an hour into the incident the patient was extricated and Rescue One was special called to assist in transporting the patient with members of Rescue Two to Rhode Island Trauma Center. The two right lanes were closed for several hours while accident reconstruction processed the scene. The patient who was from Boston died from his injuries shortly after arriving at the hospital. Fire Destroys The Vacant Morris Novelty In Pawtucket

Shortly before 08:30 on Monday Night Pawtucket Fire Alarm dispatched a full assignment to behind the Maaco Auto Body Shop at 501 Main Street for a reported structure fire. Numerous calls flooded into the Police & Fire Departments as a rescue crew transporting a patient to Memorial Hospital reported a smoke condition. Engine Two arrived within minutes and reported a "code red" with heavy smoke showing from the second floor towards the rear of the old Morris Novelty at 523 Main Street. Battalion Three arrived and established incident command as firefighters stretched a attack line to begin suppression efforts on what appeared to be a deep seated fire in the center of the building. Due to the structural integrity of the building, colliers mansion conditions, lack of egress and no life hazards reported the order was given for fire ground operations to go defensive and the interior crews were pulled out. Companies began to remove the plywood from the heavily boarded structure and the light to moderate smoke condition quickly changed to heavy black smoke that could be seen for miles throughout the surrounding Blackstone Valley. Heavy fire began to self vent from side's one, two & four causing a severe exposure problem to the one story 6,570 square foot commercial auto repair garage that was previous used by Sterry Street Towing located at 531 Main Street. Two ladder pipes and three deck guns were put into operation and began to knock down the overlapping fire and contain it to the three and four story building of origin in about an hour. Eventually a third alarm was struck and all city fire apparatus responded to the scene and Metro Control struck a box for station coverage. The fire building constructed of brick on the three lower levels and wood on the top floor which was also vinyl sided was heavily damaged and will most likely be torn down by a wrecking crew. No injuries were reported during the event and the Providence Canteen dispatched both units to the scene to provide rehab. Firefighters remained on the scene into Tuesday afternoon dousing trapped hot spots caused by the numerous collapses that occurred. The cause of the fire is under investigation by Pawtucket Fire Prevention, Pawtucket Police & Rhode Island State Fire Marshal and no further information is available. Morris Novelty was a highly popular costume and practical joke shop and was in operation for over twenty-five years, Prior to that the mill was occupied by the Biltmore Furniture Company. About ten years ago a late night arson fire heavily damaged side three of the building when a dumpster was ignited and extended into a wooden loading dock and storage area. That night Engine Two the first due company had mechanical failure and responded in the old chiefs car which was a ford station wagon. That fire was held in check by the sprinkler system without extension to the main building, The company reopened later that year. In recent years the building sat vacant and was cited by city officials numerous times for safety violations and a lack of a fire detection systems, the sprinkler system that saved the building from the previous fire was shut off and wasn't available during the latest incident hindering operations.

The last major mill fire in the city occurred about a block away at 125 Goff Avenue when the vacant "Union Wadding Company" burned in a seven alarm arson fire on Wednesday, October 13, 2010.

Just before 03:30 on Sunday Morning Pawtucket Police received a call from a resident on Kossuth Street reporting a Motorcycle Accident on Route 95 Northbound seconds later Pawtucket Fire Alarm dispatched Engine's 2,5 and Rescue 2 to the area between Exit 28 (School Street) & Exit 30 (East Street) to investigate the report. Pawtucket Police arrived within minutes and confirmed a motorcycle down with the rider unresponsive, possibly not breathing and the rescue was expedited. Rhode Island State Police arrived and along with city units began to shut down the highway at Exit 29 (Broadway & Cottage Street.) Engine Two, Engine Five & Rescue Two transported a patient with severe trauma to Rhode Island Trauma Center in Providence where he was pronounced deceased shortly after arrival. The roadway remained closed for about an hour while accident reconstruction began the investigation.

In the days following the accident several friends and family members gathered in the grassy area along the exit ramp and broadway bridge which was the site of a fatal car accident on December. 30, 2005 and established a memorial however due to safety concerns the Rhode Island State Police responded and cleared out the area. In the early evening hours of a rainy Sunday night as hundreds gathered for Randolph’s Independence Day Festivities a female driver interrupted the Parade by driving through it with her vehicle. Randolph Police arrived and gave chase while the driver entered onto Route 93 and proceeded towards Route 95. Massachusetts State Police became involved in the chase near Norwood and setup stop sticks miles South of the pursuit. With speeds in the ninety's the subject somehow missed the spike strip and continued southbound the Massachusetts State Police Air Wing was subsequently requested for air support. The pursuit traveled through Norwood, Canton, Walpole, Foxborough and exited Norfolk County in Sharon it continued south into Bristol County and went through Mansfield, North Attleborough & Attleborough the decision was made to terminate the chase at the Rhode Island state line. Rhode Island State Police were notified of this fact and troopers engaged the chase in Pawtucket at Exit 30. Due to road conditions and the drivers excessive speed she made it approximately a mile into Rhode Island and crashed in the infamous Pawtucket S-Curve after striking a light pole and the Exchange Street bridge abutment. With guns drawn Randolph, Massachusetts State, Rhode Island State & Pawtucket Police approached the vehicle. The single occupant was found unconscious and was removed and transported via Pawtucket Rescue crews to Rhode Island Trauma Center for treatment of unknown injuries. The Pawtucket S-Curve is treacherous at normal speeds and conditions and has been the site of hundreds of accidents some of them fatal. No additional injures were reported.

The Woonsocket Rubber Company was incorporated in 1867 operating from a mill building at 60-82 Main Street near Market Square, nine years later the company was the countries leading supplier of high quality rubber footwear. In 1889 Joseph Banigan the companies founder built a four story 217,689 square foot building on 7.44 acres along the Blackstone River on Fairmount Street it was named after a family member and was the largest rubber mill in the world at that time. Three years after the mill opened Woonsocket Rubber was bought out by the United States Rubber Company. The operations at the facility continued until the great depression hit in the 1930's and the mill closed leaving 1,500 jobless. The mill reopened years later and manufactured rubber good and supplied them to the efforts during the second world war before closing again in the 1960's. In September 2003 the mill was purchased by Portola Tech International a company that produces containers for various cosmetic companies and operated until 2010 when the company moved operations to 35 Martin Street in the Berkeley Mill Village of Cumberland. In 2011 the building was purchased and was in the process of being renovated for the American Wood Pellet Company. 122 years after the mill was built around 7:30 on a hot and humid Tuesday evening Woonsocket fire control dispatched a full complement consisting of Engine's 3,1,2 Ladder 2 & Chief 1 to a "Box Alarm Of Fire" at 85-87 Fairmount Street. Firefighters arrived on scene within three minutes and had "Nothing Showing" from the exterior. Crews made entry and began to investigate the cause of the alarm. As Chief One arrived he requested an emergency evacuation of the building and requested all remaining city apparatus to the scene as heavy smoke became visible from the upper floors. Multiple alarms were transmitted in quick succession requiring a massive mutual aid response from over a dozen communities in Rhode Island & nearby Massachusetts the request was filled through Northern Control. Incident command ordered incoming units on the task forces to focus efforts on exposure protection along River & Water Streets due to radiant heat from the inferno. A heavy column of smoke was visible from as far away as Providence while Oakland-Mapleville arrived and setup draft operations on the same spot along the river they used for the previous "Seville Dye Fire" earlier this year. Burrillville crews provided the adequate water supply needed for the operations along side four however a severe brand problem ignited several brush fires along the rail road tracks.
Woonsocket Tower Two, Woonsocket Reserve Ladder One, Millville Ladder One, Bellingham Ladder One, Smithfield Ladder One, Blackstone Ladder One, Pawtucket Tower One & Lincoln (Albion) Ladder 20 setup and flowed water which kept the fire from extending to additional buildings however fire extended into and destroyed the one story stand alone brick building on side one along the river. Firefighters were able to keep fire from extending to the 6,731 square foot two story office building which was also built in 1889 but the building of origin was a total loss and collapsed leaving an extensive overhaul operation that went into the following days. The cause of the fire is being called "Accidental" and was possibly caused by work men using a welding torch. Two firefighters were injured and were transported to a area hospital for further treatment, three families were also displaced for a short time and was assisted by the American Red Cross. A significant power outage also occurred and city residents were urge to conserve water in the following days due to a lack of sustainable water. Glocester (Chepatchet) Engine 13 extinguished a fully involved vehicle fire on Arnold Street at the height of the fire later in the evening a call came in for a vehicle in the water at Cold Spring Park off Harris Avenue the call was handled by Blackstone crews. The closest firehouse "Station Six" is located at 504 Fairmount Street about five blocks from the fire however was closed in 2009, Ladder One which would have been the first due ladder company responded when staffed with off duty personnel.

Early Morning Vehicle Accident In Pawtucket

In the early morning hours of Monday as the residents in the river view section of the city slept a lone vehicle traveled down School Street at a high rate of speed. Shortly before 2:15 The usual sounds of the overnight was shattered by a loud crash followed by exploding transformers, blue flames and arcing wires, a subsequent power outage ensued. I arrived at the School Street curve onto Beverage Hill Avenue about two minutes after the "bang" with a vehicle into a pole which was resting on a retaining wall and guide wire. As i approached the vehicle airbag dust poured from the car, when the smoke cleared it was apparent that the vehicle was unoccupied. About ten minutes later the police radio went off " Car One... head to the area of Prospect Heights for a loud bang." Police arrived minutes later and accessed the situation, Two additional units were requested to search the vicinity for the driver who was at that point a potential victim. Due to the integrity of the pole and the vehicle resting on the guide wire the roadway was closed at Service Road. National Grid and Verizon subsequently arrived and began to work on the pole and wiring issues. National Grid had the power fully restored to the area in about two hours however Verizon remained on scene into daylight. The curve where the incident happened has been the site of numerous accidents some of which have been fatal.

Once extrication was complete Pawtucket Rescues One & Two transported the more serious patients while Providence Rescue Three & North Providence Rescue One transported the patients with injuries of unknown extent to area hospitals.

Fatal Overnight Drag Race In Providence

Shortly before midnight on Sunday night Providence fire alarm dispatched Engine 2, Ladder 7, Special Hazards, Rescue 3 & Battalion 3 to in front of the AAA building at 111 Royal Little Drive for a motor vehicle accident with entrapment and a possible auto fire. Firefighters from the Branch Avenue station arrived within minutes amd confirmed a single vehicle head on into a metal power supply pole and one occupant trapped. Special Hazards arrived and began to extricate the aided who was removed and transported to Rhode Island Hospital by Rescue Three and members of Ladder Seven. The driver succumbed to the injuries sustained in the crash and died a short time after arriving at the hospital. The crash appears to be a result of a drag race which occurs frequently during the weekends on Royal Little Drive and also across the city on Ernest Street which was also the scene of another fatal drag race which occurred on May 28, 2006 and killed three people when their vehicle struck a building.

Shortly after eight on Wednesday morning Rehoboth "443" dispatched stations to a possible explosion & fire at the G&W Foundry building at 128 Bay State Road. Heavy smoke billowed into the morning air while members responded shortly thereafter first arriving companies found the large one story brick & wood "L" shaped commercial building heavily involved. The incident commander requested a second alarm structural assignment and the third alarm for tankers the request was filled by Bristol County Control as a command post and staging area were setup at the neighboring Blanding Library. Due to a lack of hydrants in the area Seekonk engine three arrived and began drafting from Village Pond to maintain some sort of water supply at the scene. The chief arrived and requested a Tier One Hazardous Material Response due to unknown chemicals involved along with the Massachusetts State Fire Rehab & Incident Support Units & Providence Canteen. Firefighters from Seekonk, Swansea, Plainville, Dartmouth, Somerset, Attleboro, Westport, East Providence, Warren, & Raynham work to extinguish the fire which they did in about two hours. The building was a total loss and collapsed, a local wrecking company arrived to assist in an extensive overhaul operation that went into the evening. No injuries were reported during the fire however several area schools were locked down due to the smoke. The fire is under investigation but is believed to have been caused by one of the employees mixing two different chemicals.

Shortly before 05:30 on Tuesday evening a passing Pawtucket Police officer reported smoke coming from a residential structure on Broadway and reported people trapped inside, Moments later the fire department dispatched companies to 681-683 Broadway for a reported structure fire. Due to numerous calls flooding into the fire alarm office and police on scene confirming a fire the fill in companies were dispatched. Engine Four arrived and issued a "Code Red" with heavy smoke and fire showing from the top floor on side two of the three story tenement and also requested a rescue due to a patient down in the roadway. Arriving fire companies began to stretch lines and had to knock down heavy fire conditions on the top floor in order to preform searches for the missing occupant. The incident commander special called an additional engine and multiple rescues as heavy fire began to vent from side four. About ten minutes in interior crews began to make progress on the overlapping fire, during this time members from Engine Two found an unconscious male and removed him via side four to awaiting medical crews. With all city companies on scene a mutual aid box was struck thru metro control for coverage with Providence, East Providence, Central Falls & North Providence filling in at empty stations. The heavy fire was knocked down in about an hour leaving an extensive overhaul operation that went into the evening. Three patients were transported to area hospitals for further treatment however the patient who was removed was pronounced deceased. The cause of the fire is under investigation by the fire prevention office. Later in the evening fire crews were twice again called back to 681 Broadway the first was for a intoxicated individual and the second time was for a person who received a Minor stab wound during a robbery that occurred in front of the "Super Dooper Pizza" further up the street.

The last fatal fire in the city occurred on Saturday, March 7, 2009 and killed a third floor resident at 160 Meadow Street and on November 5,2010 at 119 Englewood Avenue a small fire in the basement lead to the discovery of a deceased tenant on floor two, it is believed that the victim died prior to the fire of natural causes. Additionally on September 29, 2008 the occupied tenement neighboring the building involved in today's incident on Broadway was struck by a vehicle.

Double Fatal Overnight Fire On Smith Hill

At approximately 01:30 on a rainy and windy Monday morning South Providence fire companies were dispatched to 1251 Elmwood Avenue in the South Elmwood section of the city for a possible house fire. Engine Eleven arrived first due minutes later and reported a "Code Red" with fire in a two story wood frame duplex. Firefighters stretched two hand lines which extinguished the room and contents fire in about ten minutes, the fire was held to the first floor without further extension. About an hour later shortly after 02:30 Providence fire alarm began taking numerous calls for a fire at 25 Camden Avenue in the cities Smith Hill section. Rescue Six reported a heavy smoke condition from Roger Williams Hospital, Shortly thereafter Engine Twelve arrived and confirmed a "Code Red" with a two and a half story wood frame fully involved. Engine Seven arrived and transmitted a second alarm with heavy fire severely threatening a residential structure of similar type on side four. Tower Ladder One who responded initially on the fill in assignment as the "FAST" Truck was put to work and ordered to setup for exterior operations. Battalion Three arrived established command and reported the incident doubtful will hold with heavy fire in possession of all floors. Firefighters worked big lines from all sides and began to make headway on the lower floors in about a half hour, a heavy fire condition remained in the attic and eventually caused the chimney to become structurally compromised and fall into the building. Due to the chimney collapse heavy fire began threatening the exposure two building which is a three story vacant dwelling under construction. Once the heavy fire was knocked down members reentered and began searching the third floor for two residents who were unaccounted for. The search concluded as two female victims were found on the third floor towards the rear stairwell. Two firefighters received injuries of unknown extent, no other injuries were reported however several canines were also found deceased on the lower floor which was vacant. The cause of the fire is under investigation by the Providence Fire Prevention Office & Rhode Island State Fire Marshals Office but is being called "Suspicious." The tenement house was a total loss and subsequently demolished the following day once the investigation had concluded.

The last double fatal fire on Smith Hill occurred in the early morning hours of Monday, February 5, 2007. A Father & Daughter died when heavy fire took hold of their apartment at 687 Chalkstone Avenue.

On Friday morning at approximately 01:00 Battalion Four and all apparatus responded to 65 Shawmut Avenue for a "Investigation." Companies arrived and discovered a smoldering fire in a dwelling caused by a heating appliance. Engine One was returned to service and headed back to the station. Shortly after 01:30 police dispatch reported a possible structure fire at 44 Hedley Avenue, shortly thereafter police officers arrived at 41 Hedley Avenue and confirmed a fire towards the rear of the building. With only one engine remaining in service battalion four requested Pawtucket to fill in on the first alarm with a Engine & Ladder as Engine One arrived and reported the rear porch fully involved moments later. The second alarm was transmitted on the arrival of the chief bringing in additional mutual aid from Valley Falls, Lonsdale, Saylesville & Lincoln Rescue. Providence and North Providence provided station coverage and responded back to the Shawmut Avenue incident on another investigation. Heavy fire extended to all floors and took full possession of the attic of the large two story residential structure forcing members to go into a defensive operation. The exposure four building at the corner of Perry Avenue was evacuated during the height of the event. Central Falls Ladder One & Pawtucket Ladder Two flowed water while other members used big hand lines from all sides which contained the fire to the building within an hour. The building sustained substantial damaged and displaced several residents, the red cross responded to aid the displaced. The Providence Canteen was on scene providing fire ground rehab to the members and with temperatures in the single digits it was greatly appreciated. One firefighter received injuries of unknown extent and was subsequently transported to a area hospital for treatment. The cause of the fire is under investigation the state fire marshals office.

As heavy fire conditions took hold of the river side of the complex crews flowed water from Tower Ladder Two, Valley Falls Truck One & Smithfield Ladder One along with numerous deck guns which protected the larger two story section from becoming involved. Oakland-Mapleville & Nasonville crews arrived and began drafting from the Blackstone River to maintain a water supply for the crews with large stream devices on side four. Due to the heavy smoke and potential for hazardous materials burning inside the vacant manufacturing facility several area homes were evacuated. the red cross responded to aid the displaced. Firefighters from Bellingham, Burrilliville, Millville, North Smithfield, Smithfield, Uxbridge, Blackstone, Central Falls, Cumberland & Lincoln worked for hours to contain the fire to the building of origin. The incident commander placed the fire under control at 05:56 that evening however crews remained on scene throughout the following week dousing hot spots. No injuries were reported in the event, The providence canteen responded to provide rehab and refreshments to the members and displaced residents. While en route car three was notified of numerous calls reporting the fire and possible people trapped on a upper floor fire escape and a additional engine was added to the response. Engine four arrived shortly thereafter and reported a fully involved structure fire and began fire suppression efforts, Car three arrived and reported a heavily involved three story wood frame and transmitted a second alarm. A heavy fire condition vented from sides one & four on the first floor and firefighters setup for a defensive operation. Fire extended to all floors and thru the roof forcing members to gain control with exterior lines setup on all sides, ladder two's pipe also flowed from side one. Firefighters began to make progress about two hours into the incident leaving an extensive overhaul operation that went into early Wednesday morning. Several residents suffered minor injuries jumping from the upper floor, additionally during overhaul a second floor resident was found deceased feet from the first floor entry way. The cause of the fire is currently under investigation however doesn't appear to be suspicious in nature. The building was completely destroyed and will be raised by a local wrecking company.

Late on Thursday night crews were once again dispatched to a reported fire this time at 120 Diman Street, Engine three arrived with the top floor of a large three story tenement fully involved. The fire was controlled with an exterior operation in about an hour leaving the building a total loss.

Arson Fire Destroys Pawtucket Tenement

Shortly before 11:00 on Wednesday Morning Pawtucket fire alarm dispatched Engine's 1,2,5 Tower Ladder 1 & Battalion 4 to 35 Vale Street for a structure fire "Smoke In The Building." Engine One arrived within minutes and reported a "Code Red" with a fire in a three story six unit apartment building and reported possible people trapped. Engine 3, Ladder 2 & Rescue 1 filled in on the code red assignment. A heavy fire condition on floor two rapidly extended into the third floor and command requested an additional engine company to the scene. Shortly thereafter the chief requested another special call for engine six who arrived, dug out a hydrant and established a water supply for the tower ladder. With all the city apparatus at the scene mutual aid box 2826 was transmitted for station coverage with North Providence, Providence & East Providence Responding. Heavy fire took hold of the upper floors forcing command to evacuate the building. Tower Ladder One began to flow water as did Engine One who dumped the deck gun from side two, Ladder Two set up on side four and flowed water to protect the exposure a 1.5 story wood frame. Crews had the heavy fire knocked down in about a hour however the building was a total loss. The fire is believed to be a case of arson and is possibly connected to a home invasion case which occurred at the same address On December 6th 2010. Raymond Gyamfi is still at large and wanted in regards to that case. Tony Reverdes and Joao Dosantos other suspect were subsequently apprehended and are in custody, Reverdes was previously convicted of first degree arson among other things. Battalion One reported a code red and requested an additional ladder company followed shortly thereafter by the transmission of a second alarm. Additional crews arrived and began forcing entry and stretching lines searching for the seat of the fire as the smoke condition from the exterior worsened. Providence Engine 12 who was operating in the basement reported fire in the woman's locker room which appeared to be gas fed. Incident command requested two additional engine companies and another rescue due to a injured Providence firefighter on side two. On scene crews began to make headway once the fire was uncovered and crews had the heavy fire contained and knocked down in about a half hour. Members then began to check for extension and engage in an extensive overhaul operation that went into the evening, the injured member was subsequently transported to Fatima Hospital with injuries of unknown extent. The mutual aid departments started to be released from the scene in about an hour. The cause of the fire is being investigated by the state fire marshals office. The recall was sounded at 32 minutes after midnight.

At 2:18 on Tuesday afternoon a passing fall river district chief noticed smoke from the area of Bay & Middle Streets and reported a possible vehicle fire. Moments later fire alarm dispatched a full still alarm to 1 Middle Street Duro Textiles Plant Number Two for reports of a fire. Shortly after arrival it was apparent that the incident was more than a car fire and additional crews were requested which balanced out to a second alarm equivalent. Heavy fire conditions took hold of the old boiler room area which was setback along the train tracks behind the main mill building and began extending to the roof of Building Four and the soffit area of the larger section. Firefighters engaged in an interior operation in surrounding buildings as additional members setup ladder pipes and played exterior lines via surrounding roofs onto the building of origin. Police officers evacuated the Matthew J. Cuss Middle School and the East Gate Christian Academy Day Care Center during the height of the fire. Eventually command requested a Fourth alarm which brought all city apparatus to the fire in addition to about 50 recruits also requested was a Bristol County Structural Task Force which brought in mutual aid from Swansea, Somerset, Tiverton, Westport, Portsmouth, Dartmouth, and Seekonk a fire boat was also on scene but not used. Heavy fire burned for hours causing numerous collapses of the three story building as members tried to gain control which they did shortly before six in the evening. Two firefighters were injured in the fire and were transported to a local hospital. This was the first general alarm fire in the city in about ten years. The cause of the fire is still under investigation by The State fire marshal, A.T.F and Fall River Officials but is being called suspicious. Duro Textiles is the worlds largest distributor of woven and non woven textiles and also manufactures camouflage clothing for the armed forces for use overseas, Plant two has been vacant for a couple years and was in the process of being boarded up and secured to prevent youths and vandals into the structure.

In 1847 two men Darius Goff and George Lawton bought land south of the railroad tracks and built a cotton wadding mill which they operated until it was destroyed by fire in 1851. The two then rebuilt the mill building but had a falling out in 1859. Darius Goff then partnered himself with John Cranston and Stephen Brownell and formed the company "Goff, Cranston & Brownell" who operated the mill building dealing with the buying and selling of cotton waste from other area mills and the manufacturing of Cotton Batting, Cotton Wadding and Paper Stock. In early 1870 the mill was destroyed by fire and rebuilt covering a area of three acres. Seven years later in 1879 the three changed the name of the operation to the "Union Wadding Company." The storehouse building was built two years later in 1881, and was destroyed by a fire caused by a lighting strike on July 31,1898. Shortly after 11:30 on Wednesday, October 13,2010 multiple calls began to be received at Pawtucket fire alarm reporting a building fire at 125 Goff Avenue. Engine Two left the station and transmitted a third alarm with heavy smoke in the distance and arrived at the mill with heavy fire from the rear of the still vacant section. On arrival of the battalion chief a fourth and fifth alarm were transmitted bringing in a massive mutual aid response. Heavy fire took hold of numerous interconnected buildings ranging from one to three stories in height. Numerous collapses occurred throughout the incident sending up a loom that could be seen for miles and causing several exposure problems to adjacent buildings one of which is Roberts Chemical on Pine Street. Firefighters from Central Falls, Cumberland, Lincoln, Attleboro, Seekonk, North Providence, Providence, East Providence, Cranston and Johnston worked in a surround and drown operation to knock down the overlapping fire, crews began to make progress later into Thursday morning. All of the occupants who occupy the loft apartments were safely evacuated with no injuries reported. The fire is currently under investigation by the Rhode Island Fire Marshal, Pawtucket Fire Prevention. ATF & Pawtucket Police. Firefighters remained on scene into the weekend working with hot spots still burning in the voids left during the collapses.

Double Fatal Car Accident In Pawtucket

Shortly before four thirty on Sunday afternoon Pawtucket fire alarm dispatched Engine Two & Rescue One to Weeden & Barton Streets for a motor vehicle accident, car verses tree. Numerous calls were being received reporting possible entrapment and Ladder One & Battalion Three were dispatched to assist in a potential extrication. Engine two arrived in front of the Furniture Warehouse at 329 Barton Street and confirmed a vehicle into a tree with three patients and special called two additional rescues bringing in Pawtucket Rescue Two and Central Falls Rescue One and Engine Five for manpower, A fourth patient fled the scene on foot. All three patients were transported to the trauma center at Rhode Island Hospital where two later died, the third person is still in serious condition. The Pawtucket Police accident reconstruction and Crime scene unit remained on the scene into the evening investigating the cause of the incident. Hours later the fourth person was located on Cowden Street in Central Falls and was transported to Rhode Island Hospital with injuries of unknown extent.

This Was also the second fatal car accident in the city in about two weeks. Shortly before midnight on Saturday, September 18 a Massachusetts man was killed following a rollover at the intersection of Power Road and Mineral Spring Avenue, several others were injured.

Overnight Third Alarm In Central Falls

On Friday evening as a majority of the area was being inundated with calls due to heavy rains and wind from the remnants of tropical storm Nicole, a call came into the Central Falls fire alarm office reporting a possible structure fire, At 22:25 Battalion 1 and all apparatus responded to 88 Sacred Heart Avenue which runs from Broad Street to High Street to investigate the report. Engine Two arrived minutes later and reported fire showing from the top floor on side four of a large four story occupied woodframed building and initiated an interior operation. On arrival of the battalion chief a second alarm was transmitted bring in mutual aid from Pawtucket, Lincoln & Cumberland. Pawtucket Ladder two arrived and set up in front of 142 Railroad Street which is the first floor of the fire building that sits below grade on side two and crews assisted in stretching an additional handline over the stick to members of Pawtucket Engine two who were on the third floor. Heavy fire extended from floor three into the cockloft and thru the roof shortly thereafter a third alarm was transmitted with a special call for the Cumberland air supply. When the heavy fire was knocked down and members were engaged in an extensive overhaul operation a mayday was transmitted for a downed firefighter on floor three, Saylesville Ladder 51 who had assembled as the RIT team on side one began to head in but stood down as members from Central Falls & Pawtucket assisted the firefighter who received a shoulder injury out of the building to the awaiting Cumberland rescue crew. A second firefighter was also injured later into the incident followed by a Pawtucket firefighter who received transport to a area hospital after returning to quarters. The cause of the fire is currently under investigation, The recall was sounded at 02:12.

Shortly after 03:30 on Monday afternoon Woonsocket fire control dispatched a full assignment to Center & Olo Streets for a reported fire. As luck would have it i was at the traffic light for South Main & River Streets and made the block trip to the reported job. First due companies were greeted with fire showing from the second floor window on side two of the occupied 2.5 story woodframe and began to stretch a line. Chief one arrived confirmed the "Code Red" and transmitted the second alarm signal bringing in additional crews as heavy smoke pushed from the rear of the structure. Interior crews made quick work having the visible fire on floor two knocked down in about ten minutes crews chased pockets of fire into floor three and the attic space before being placed completely under control. No injuries were reported during the incident, The providence canteen also responded to provide rehab. This is the third time in about a week that a multiple alarm fire has damaged a tenement house in the city. On July 18th a two alarm fire destroyed a three story occupied building at 112 Gilbert Street and on July 23rd a three alarm fire damaged a 2.5 story occupied dwelling at 86 Park Avenue the cause of today's fire is currently under investigation.

Shortly after 01:45 on Saturday morning a Pawtucket Police officer on routine patrol noticed a large column of smoke rising from the area of Pawtucket Avenue & Main Street and began to investigate. The officer arrived at the Pep Boys located at 1246 North Main Street and reported a structure fire, During this time Providence fire alarm dispatched Engine 2 to the Pep Boys & Engine 4 to North Main & White Streets for dumpster fires. Engine two arrived first due at a dumpster fire against a vacant commercial building at Nashua Street and Smithfield Avenue and stretched one line while Engine four arrived further up the block and went to work on a pile of tires and other rubbish inside a storage shed which had extended to shrubbery next to the building. The fires were contained with no extension to any structures within ten minutes. The cause of the fires are currently under investigation.

Shortly after 12:30 on Wednesday afternoon Pawtucket fire alarm dispatched Engine's 1,2,5 Tower Ladder 1 & Battalion 4 to a reported building fire at 288 Lafayette Street in the cities west side. Car 4 arrived on scene within seconds and reported a "Code Red" and Engine 3, Ladder 2 & Rescue 1 were dispatched to fill out the assignment. Engine One arrived and transmitted a second alarm bringing Engine 4 & Rescue 2 to the scene as members stretched a attack line in via side one. Heavy fire vented from the window on side four while Tower ladder one arrived and setup to began roof ventilation operations during this time a female occupant was reported missing to the crew of Engine two who began an aggressive search to follow up on the report. Heavy fire extended due to balloon frame construction via the side four wall into the half story and attic space, During this time numerous firefighters suffered heat related injuries and went to rehab. The incident commander special called Engine 6 who cleared a vehicle fire on York Avenue, Central Falls Engine 2 who was on the cover assignment and Providence Rescue 3 which brought the last remaining city fire company to the scene and balanced out to the equivalent of a third alarm assignment. Interior crews uncovered heavy fire conditions between the first and second floor and the attic which forced the evacuation of the building. Crews went to work with exterior lines and a ladder pipe as heavy fire vented thru the vent hole atop the roof and thru the side one gable vent. Following a defensive operation that lasted about a half hour members reentered the building to further overhaul and check for the cause of the fire which is believed to have been intentionally set, the occupant who was initially reported missing was located safely at a different location.

Around 03:00 on Sunday Morning Providence fire alarm dispatched Engine's 12,2,7 Special Hazards Ladder's 3 & 7 Rescue 3 & Battalion 3 to the area of Veazie Street for a walk in at the Admiral Street firehouse reporting a vacant house fire. Engine twelve arrived at 31 Veazie Street and made it a code red with a exterior fire against a three story brick & wood constructed building. This fire is in the same general area as the three arson fires on Douglas Avenue which occurred on Thursday, May 27, 2010 in the same manner. As i made my way to the fire there was a visible column of smoke from the area of cove metals located at 160 Grenville Street in Pawtucket, The company is often the culprit of "Smoke In The Area" calls and i didn't think to much of it. As I passed the walgreens on Admiral Street Battalion Three reported to fire alarm that they were holding Engine Twelve & Ladder Three shortly there after Engine four was dispatched to the Chili's Restaurant at 255 Collyer Street for a vehicle fire. On arrival Engine four found a fully involved van and stretched a line to extinguish the fire. Members had the fire quickly knocked down fire also damaged a picnic table which was in front of the vehicle. About five minutes after that fire was extinguished Pawtucket police found another vehicle fire at Waldo & Clyde Streets. Engine One under command of Captain Thurber arrived minutes later and made quick work of the fire. As crews pickup from the second car fire Rescue one was dispatched to the Hess Gas Station at Lonsdale & Mineral Spring Avenues for a man who lit himself on fire, the patient was transported to Rhode Island Hospital for further treatment.

Around 6:40 on Monday night East Providence fire alarm dispatched Engine's 5,1,3 Ladder 3 & Battalion 1 to Cross Street At Summit Street for a reported structure fire called in by police who were on scene. As crews responded Battalion One was notified of numerous calls being received and the balance of the working fire assignment was dispatched out. Engine Five arrived on scene and reported a fully involved structure and command requested the last remaining city apparatus to the scene for staging shortly thereafter a mutual aid box was filled out for station coverage. Heavy fire of the first and second floors of the large 1.5 story three bay garage extended thru the roof before being knocked down and held to the building of origin in about an hour. No injuries were reported during the fire which heavily damaged the building. Smoke from the fire drifted for miles and caused numerous calls for smoke in the area in neighboring Pawtucket.

In The late afternoon hours of Monday, June 07, 2010 a fire was reported in the vacant mill building at Mechanics Street (Route 12) and Furnace Street. The building formerly known as the "Putnam Foundry" was heavily damaged by a general alarm fire on August 31, 2007 and has also been the site of several other arson fires since and is in various states of decay. The chief arrived on scene and reported a working mill fire and setup exterior lines for fire attack. Crews arrived and went to work stretching a supply line from Route 44 to apparatus on side one and also from across mechanics street down Furnace Street to supply apparatus in the rear of the building. The fire was knock down within an hour without any extension to the other vacant buildings on the property. No injuries were reported during the incident which is currently under investigation. Engine three out of the Columbus Avenue firehouse arrived first due and reported a "Code Red' with fire showing in the second story windows. Engine 2 Tower Ladder 1 & Rescue 2 filled out the balance of the assignment. Members forced entry and stretched in lines to begin suppression efforts as additional crews worked to extinguish fire extending up the exterior of the building. Incident command requested an additional engine company and Engine 1 who was responding to Star Street for a alarm bell sounding diverted to the fire, Shortly thereafter an additional special call was made for Engine 5 to respond and setup as the RIT company. The mutual aid box was requested out of metro control and station coverage was provided by surrounding communities as crews on the fireground stretched a line to the tower ladder in preparation for a defensive attack. Interior crews made progress on the overlapping fire on floor two within twenty minutes but fire extended into the attic space and eventually thru the vent hole on the roof before being controled. No injuries were reported during the fire but one canine perished, the cause of the fire is still under investigation by the fire prevention office. The recall was sounded at one minute after midnight.

Approximately two hours after clearing the police detail requested the fire department for a small rekindle in the soffit area above the front door of the heavily damaged building, the fire was quickly knocked down using one line.

Shortly after 22:00 on Friday evening Fall River Fire alarm dispatched a "Still Alarm" to 420 Quequechan Street "The Wampanoag Mill Grill" for a reported Odor of rubber burning. Car two arrived and reported fire showing from the rear of the building, moments later he reported the former Parker Candy Company fully involved. A defensive operation was put underway as heavy fire burned throughout the first floor of the two story granite mill type structure. Firefighters setup on all sides to protect the large multiple story mill type buildings which surround the building which was burning now thru the roof. Members chased pockets of fire into the next day when a wrecking company arrived to level the heavily damaged building. The vacant Parker Candy Mill Building was one of the sites of a four hour arson spree which plagued the city of Fall River in the early morning hours of June 29, 2008, the man responsible was arrested and charged with numerous counts of arson. At approximately 17:00 on Saturday evening as the final fire companies were clearing the fire scene another still alarm was reported at 5500 North Main Street with police on scene reporting "Smoke Showing." Engine 12 arrived and went to work with a fire in a floor one apartment at the four winds apartment complex. The fire was quickly knocked down with no injuries reported.

A week later On April 24, 2010 around 08:00pm crews were dispatched out to a "Still Alarm" at the former Cardi's Furniture warehouse at 638 Quequechan Street. Crews arrived to find fire showing on the exterior extending into the roofing of the vacant mill type building and upon further investigation several incendiary fires were found inside the building and a second alarm assignment was requested, the fires were held in check and knocked down by the sprinkler system. As crews were checking for any additional fires fire alarm notified command of a Box Alarm coming in for the Tower Outlet Mills located at 657 Quarry Street almost directly across from the first Cardi's and another first alarm assignment was dispatched. The first due engine company reported "Smoke Showing" from the basement of the mill and confirmed multiple set fires at that location which were being contained by sprinklers. Shortly thereafter police arrived at 420 Quequechan Street, the Wampanoag Mill and reported a tractor trailer truck fully involved extending to the loading dock and yet another full first alarm was dispatched out. Police also uncovered mattresses burning in the hallway of the Flint Mills On Alden Street that fire was quickly put under control by a Tiverton engine as well as a brush fire across from the leveled Parker Candy Mill which had burned a week prior. The entire area became a crime scene and police later arrested a man who was recording the fires and have charged him with the arson spree.

As crews were engaged in the search, one member became trapped on floor three and was forced to exit via a ground ladder on side two. A Woman and child were removed and transported via ground to St. Annes Hospital where the child later died. Heavy fire conditions took hold of the building forcing a exterior or defensive operation to be put forth to knock down the fire and protect the exposures along Manchester Street. During roll call one firefighter was unaccounted for. Members from Ladder four setup as a RIT Team and entered the heavily involved building and preformed a aggressive search for the potential missing member but was forced to evacuated once again as conditions worsened, The missing member was located safely a half hour later at a different location. Two ladder pipes and numerous exterior streams had the visible fire knocked down within two hours. The building was destroyed, Numerous injuries were reported.

Interior crews located a third victim who was unconscious and badly burned and brought him out via side two to awaiting rescue crews, an extensive resuscitation effort was put underway but tragically the man was pronounced deceased at Rhode Island Hospital blocks away. At 22:46 On Sunday, February 7, 2010 Pawtucket fire alarm dispatched Engine's 1,2,5 Tower Ladder 1 & Battalion 3 to the vicinity of Main & Magill Streets for a report of "smoke in the area." Engine one arrived in the area with a heavy odor of rubber burning and advised incoming companies to check the Sisson and Knowles Street areas, Rescue one who was in quarters on West Avenue advised Battalion three of a heavy odor and slight haze in that vicinity shortly thereafter the Captain of Engine Two advised command that the wind was blowing from across Route 95 and Engine five was sent to investigate the area of the Grotto Avenue dump which has seen many fires in the past years. As crews continued to search for the source of the smoke the officer of Engine five reported a large column of smoke coming up from the area of the tyler reed building at 560 Mineral Spring Avenue, fearing the worst crews cleared Main street and headed for that direction. Engine five arrived at 1 San Antonio Way with three tractor trailer trucks fully involved in the trucking lot for M&G Transportation and began fire suppression efforts. Battalion Three reported a "Code Red for responding companies" and special called Engine three. Firefighters used a foam operation on the three vehicles and due to the lack of hydrants in the area had to shuttle water into the scene using the hydrant on the Mineral Spring Avenue side of Providence Metallizing which was the scene of a small fire the day before. The cause of the fire is under investigation of the fire prevention office, no injuries were reported during the incident which was recalled at 00:24 Monday Morning.

Firefighters stretched in lines via sides one and four and pushed the fire out side three while members checked the upper floors for any extension, Ladder 2 arrived and setup to vent the roof. Incident Command Requested A additional Engine for a RIT Or FAST Team but the requested was canceled shortly thereafter. Rescue 1 transported a patient to a area hospital with injuries of unknown extent, The fire prevention office is currently looking into a cause. The recall was sounded at 15:16.

This is the second fire of the day earlier in the morning firefighters responded to a small fire in a large mill type building in the 500 block of Mineral Spring Avenue which was the second fire in that building in less than a week, also today's fire was directly across the street from a garage fire at 810 Mineral Spring Avenue which occurred on June 3, 2009.

At 09:53 On New Years Morning Pawtucket Fire Alarm dispatched Engine's 6,3,4 Ladder 2 & Battalion 3 to 24-28 Slater Park Avenue which runs from Armistice Blvd to Waterman Street for a report Of heavy smoke On The third floor. Engine 6 out of the Newport Avenue station arrived with nothing showing from a occupied three story woodframe, Upon further investigation the officer of the sixes reported a "Code Red for responding companies" as heavy smoke was discovered pushing from the wall on side four and a line was requested. Engine 4 arrived and dropped a supply line to adequately maintain a water supply as members from Engine 3 searched floor two for the seat of the fire, Ladder 2 took the front of the building and threw the stick. Fire was discovered in a floor two closet area and quickly knocked down members stretched a back up line to floor three as interior crews began opening up checking for horizontal and vertical extension. Battalion 3 setup incident command on side one and special called Tower Ladder 1 who arrived and setup fans to evacuated trapped smoke and assisted in the overhaul operation. No injuries were reported during the fire which is under investigation by the fire prevention bureau, The special signal fire association responded to provide rehab for the members and displaced residents. The Never Enuf Baking Company Occupied The First Floor Of The Mill Building located at 444 Roosevelt Avenue on the Central Falls/Pawtucket line Since 1910 but had shut down operations in the years prior to 1996, On a hot and humid summer afternoon of that year a fire was reported in the basement of the building. Arriving companies were met with a well advanced basement fire which had extended into the second floor which was being used as residential living space for the previous owners of the bakery. The two alarm fire heavily damaged the building at that point. The large mill building has stood vacant and had been in the process of being renovated in recent years.

At 11:01 Tuesday Morning December 29, 2009 Pawtucket fire alarm dispatched a full assignment to 444 Roosevelt Avenue for a reported building fire, At 11:03 the Central Falls fire alarm Office dispatched all city apparatus to the vicinity for a fire. Pawtucket Engine Two arrived with heavy smoke from the rear of the one and two story vacant "U" shaped mill type structure and transmitted a working fire and immediately requested a elevated master stream get setup moments later Central Falls Engine Two arrived on scene and transmitted a second alarm. Firefighters from both cities worked on forcing entry and stretching in lines as members made an attempt at a aggressive interior attack. With little headway being made the incident command staff requested a third alarm and began setting up additional crews for defensive operations. Due to weather and fire conditions along with building construction and a possible truss roof the evacuation order was given followed by the quick successions of the fourth and fifth alarms. Pawtucket Ladder One, Pawtucket Ladder Two, Providence Tower Ladder One, East Providence Ladder Three & Cumberland (Valley Falls) Truck One setup elevated streams with three flowing water. Members evacuated the apartment building at 28 Cross Street due to a potential exposure problem the displaced residents were brought to a temporary shelter which was setup at St Joesph Church located at 391 High Street. Firefighters from Central Falls, Pawtucket, Providence, Attleboro, East Providence, North Providence, Cumberland & Lincoln worked for hours in a defensive operation to contain the fire to the buildings of origin which were heavily damaged. Several firefighters received minor injuries a couple of which were transported to Memorial Hospital for further evaluation. Pawtucket sounded the recall at 15:41, Central Falls firefighters remained on scene working an extensive overhaul operation that lasted until about 17:00 when companies started returning to service the recall was sounded at 18:10. This was the second fire in Central Falls in Ten Hours shortly after midnight Engine One was stilled out to 36 Rand Street for a vehicle fire on arrival the balance of a working fire assignment was requested with a fully involved car extending to the rear of a dwelling. The cause of both fires is still under investigation by the state fire marshals office.

Fire Damages Vacant Building In Woonsocket

On A Rainy Sunday Morning Woonsocket Fire Control dispatched a full assignment to Locust Street for a vehicle fire near a garage, Locust Street runs from Cumberland Street (Route 126) To Elm & Robinson Streets and is a block from the four alarm fire the night before. Engine Two arrived first due and held the incident with that crew and the crew of the Tower the deputy also requested the investigator to the scene. As crews began to return to service the radio opened up "Fire Control To IC We Have A Reported Structure Fire At 79 Robinson Street" the chief responded "10-4, Thats A Code Red" Crews still in the area arrived to find heavy smoke and fire venting from the front porches of a vacant 3.5 story woodframe and began to stretch in. Fire Control notified the deputy of a reported electrical fire on Mill Street and mutual aid responded to that incident. Crews made great progress knocking down fire which extended to all floors due to balloon frame construction within an hour.

Later That Morning Fire Control dispatched a full assignment to Mill & Robinson Streets for a reported alarm sounded Engine two arrived to find a small incendiary type fire in the basement of another vacant structure, the fire was quickly brought under control.

It Should Be Noted That There Have Been Numerous fires in the area in recent months, fire heavily damaged a one story double wide trailer at Robinson Street & Merida Avenue On July 14, 2009 and also a vehicle was intentionally set on in the days following the trailer fire it is one of many fires which have been lit in the area. Three People have been arrested and charged for the string of fires in the area and are being held at the ACI in Cranston.

Four Alarm Fire In Woonsocket On Christmas Night

Shortly after 18:30 On Christmas Night Woonsocket Fire Control announced a reported structure fire in the area of Elm Street & Gaulin Avenue. Engine Two made the left onto Cumberland Hill Road and transmitted a second alarm due to a large header, Moments later Engine 2 confirmed the Code Red as the company arrived at a four story vacant building on Gaulin Avenue. Heavy Fire conditions on the lower floors extended via the rear thru the roof and taking full possession of the cockloft. Tower Two arrived and was ordered to setup to protect the exposure two building which is another four story woodframe vacant building which was the site of a previous arson fire on August 2,2009. As fire progressed further throughout the upper floors the evacuation order was given along with transmission of the third and fourth alarms which brought in extensive mutual aid into the city. Woonsocket Ladder One & Tower Two set up elevated streams along with Blackstone Ladder One as heavy smoke banked down to the ground causing zero visibility at some points. Due to the smoke and radiant heat from the fire several neighboring occupied dwellings were evacuated forcing families out into the cold on Christmas Night. The defensive attack had the heavy volume of fire knocked down in about two hours, the roof collapsed into the third floor leaving an extensive overhaul operation which lasted into the evening. The cause of the fire is still under investigation but is believed to have been intentionally set, No injuries were reported during the event.

Shortly before 04:00AM Cranston Fire Alarm dispatched A 3&1 Response for a reported structural fire at 1031 Broad Street at the corner of Wentworth Avenue. A Second call was received reporting a possible occupant trapped on the second floor and the working fire assignment was dispatched. Engine 1 arrived shortly thereafter with a well involved upper floor of a 2.5 story woodframe and initiated interior operations. Incident command made a request for an additional engine & truck and advised tower ladder 2 to setup on side one. Command determined that little progress was being made by interior crews and the evacuation order was sounded. Firefighters used multiple handlines and a elevated stream from the tower to make quick work of the overlapping fire leaving only an extensive overhaul operation which lasted into the morning. The occupant which was initially reported missing was safely located outside the building, no injuries were reported.

Shortly before 07:00 on Friday evening a motorist noticed a large column of smoke while sitting at the traffic light at Lonsdale Ave & Weeden Street. Upon further investigation a commercial dumpster was discovered well involved with fire burning up the vinyl siding of a vacant 2.5 story woodframe at Centre & Coleman Streets. Pawtucket Engine two arrived and began to initiated suppression efforts as additional companies arrived and went to work. Two lines were run one for the exposure and one for the dumpster it appeared that jakes were able to keep the fire contained to the exterior. Tower one forced entry and checked for interior extension on all floors which proved negative.

Around 11:00 firefighters were dispatched to the area of the Chinese Church at 333 Roosevelt Avenue for a reported structure fire. Engine two arrived with "Nothing Showing" and began investigating the area. Engine four arrived at a open burn on High Street and the other companies were cleared and went back into service.

Around 03:00 the "300" tone went off again as Engine's 1,2,5 Ladder 1 & Battalion 4 were stilled out to a reported structure fire at 121 Magill Street. Engine One out of the West Avenue station arrived first due with heavy smoke and fire from the third floor of a three story occupied dwelling and began a interior operation. Fire began to burn thru the roof and incident command requested a third alarm due to the current conditions and a potential for a exposure problem on sides two and four. Heavy fire ran the entire roof line of the dwelling and the evacuation order was given, during this time the eve on side four collapsed which hindered egress. Members setup elevated appliances from Tower One and Ladder Two along with a portable gun and numerous large lines. The overlapping fire was knocked down in about a hour leaving an extensive overhaul operation that lasted several hours. The fire building was heavily damaged, The Red Cross was requested to assist the displaced residents, One firefighter received injuries of unknown extent. The cause is under investigation by the state fire marshal.

Around 06:00 Pawtucket Police received a call for a vehicle fire On Larch Street, moments later they rebroadcasted "all cars heading to larch street be advised 9-1-1 is receiving numerous calls for another vehicle fire at 10 Baldwin Street" The cover assignment Providence Engine 2, North Providence Engine 3, East Providence Engine 3 & Pawtucket Ladder 3 were dispatched to the Baldwin Street incident as Police arrived and reported the car fully involved next to a building. Providence Engine 2 arrived first in and reported a "Code Red" with fire extending to the exterior of a large 3.5 story occupied building and ran a line for extinguishment. The Magill Street incident command began releasing crews Engine's 2,6 Rescue 2 responded in. The heavy fire was knocked down in about ten minutes, crews checked for interior extension which proved negative on all floors. No Injuries were reported during this fire, The second vehicle fire on larch street was extinguished on arrival.

Early Morning Fire Levels House In Dighton

At 01:00 on Friday Morning Dighton Firefighters were dispatched to 1527 Wellington Street on the Rehoboth Town Line for a reported structure fire. Police officers arrived on scene and reported a active fire with fire thru the roof moments later Car 1 arrived and reported a two story dwelling fully involved and Requested a Rehoboth Tanker. The first due engine arrived and was forced to forward lay from the hydrant which was about 1000 feet away down a long driveway to adequately maintain a water supply. Incident command requested the balance of a working fire assignment which brought additional mutual aid from Rehoboth, Taunton, Swansea the Rehoboth ladder is on the working fire run card but canceled due to lack of accessibility. Heavy fire burned through out the building for hours, eventually the town was requested with a back hoe and fireground operations were halted to allow the Dighton ladder to get to side one and setup a elevated stream. The dwelling which was in various states of decay was a complete loss, No injuries were reported. The cause is still under investigation but is likely caused by the chimney which was constructed of multiple 55 gallon drums welded together.

Five Alarms For The "Bourne Mill" In Tiverton

At 01:48 On Saturday Morning Tiverton firefighters were toned out to the "Bourne Mill" at 1 State Avenue for a report from the police of smoke coming from a warehouse. The large manufacturing complex along Cook Pond consisted of twenty-three different buildings constructed between 1881-1951 in a area of about twenty acres and is on the register of historic buildings. The complex which is in various states of decay is in the process of being renovated and converted into living space. Shortly after the dispatch fire alarm notified crews of a possible vehicle fire also on the State Avenue as Engine 2 reported enroute with "smoke showing" in the distance and arrived on scene at 01:50 with a two & three story brick mill type building with the top two floors heavily involved and a severe exposure problem to the main building which is a five story "L" Shaped mill type structure with attached two and three story buildings in the rear. A Defensive operation was put underway due to structural involvement along with transmission of a second alarm. Numerous special calls were requested bringing in a massive mutual aid response from surrounding Bristol County in regards. Crews setup lines for exposure protection as the building of origin began to collapse, subsequently a fourth alarm was transmitted with a request for two ladders over that assignment. Ladder trucks from Portsmouth, Middletown & Fall River worked with elevated streams while deck guns and large exterior streams were put to work which began to make progress on the overlapping fire in about two hours. Crews stretched multiple lines into the larger mill building to check for extension which amazingly proved negative on all floors, but several windows were damaged due to the radiant heat. The incident commander placed the fire under control at 03:54 with an extensive overhaul operation remaining into the afternoon. The state fire marshal was requested due to the suspicious nature of the fire. One firefighter was transported with minor injuries, the Providence Canteen & Fall River Special Services responded for rehab.

Tiverton Pond Claims Another Life

On Monday, July 7, 2008 Tiverton firefighters along with multiple surrounding fire departments were engaged in an extensive search for a missing elderly boater on the Sakonnet River, the recovery operation took three days. On Sunday, August 3rd almost a month later firefighters responded to Stafford Pond off Old Stafford Road for a reported man fallen into the water. Firefighter Gerald Leduc who was appointed to the Tiverton Fire Department on May 9, 1984 responded to assist in the operation, he suffered a fatal heart attack while searching on the pond. The twenty-five year veteran became the first line of duty death for the towns fire department and was buried with full honors on Friday, August 8th. Shortly before 15:00 on Tuesday, August 4th one year after that tragic night the vocal alarms at stations 2,3 & 4 went off for another water incident on Stafford Pond. Firefighters arrived and launched a boat as one occupant from the capsized vessel made it safely back to shore and gave information to first responders. Westport, Little Compton & Fall River responded with boats along with a request for a coast guard helicopter for aerial assistance. The command post setup at the boat ramp, while boats were deployed the wreath from fallen firefighters memorial service the night before floated in the shallow water off shore. Shortly before 15:30 the Westport fire boat notified command that the patient had been found, the man was brought back to shore where he was pronounced deceased.

Shortly before 19:30 on Sunday evening Johnston fire alarm dispatched Engine's 1,3,2 Ladder 1 Rescue 1 & Battalion 1 to 216 Morgan Avenue between La Fazia Drive & Downing Drive for a reported structural fire. Engine one arrived and reported "smoke showing" from the second floor of the large 2.5 story wood frame with an attached 1.5 story section and ran a line to the interior via side one. Battalion one arrived and established command reporting a "code red" subsequently Engine 4 & Rescue 3 were dispatched to fill out the assignment. Interior companies made the second floor and reported no visible fire but a heavy smoke condition, due to the report a second line was stretched into floor one while additional members opened up in attempt to find the seat of the fire. A mutual aid box was transmitted for station coverage with North Providence, Providence & Cranston responding for coverage while members on scene reported a possible basement fire. Heavy overlapping fire began venting from the second floor of the half story on side four auto extending to the larger portion of the vacant building and eventually taking full possession of the attic space from front to back. Incident command gave the evacuation order and put five lines of various diameter to work on all sides along with deck guns from Engine's one and two which knocked the heavy fire back enough to allow reentry. Firefighters had the fire contained to the building of origin within an hour leaving an extensive overhaul operation with remaining pockets of fire burning in the eves and roofing around the chimney, The building was heavily damaged. Two firefighters were transported to a area hospital with injuries of unknown extent, the cause of the fire is under investigation by the state fire marshal.

This fire is about a mile from another fire that occurred on August 20, 2006 at 63 Downing Drive the 2 story wood frame with attached one story garage was destroyed in a early evening fire that required mutual aid from North Providence & Cranston to bring under control.

Shortly after one on Saturday morning the Boston Fire Alarm office announced "In district 6 striking Box 7218 Old Colony Avenue & D Street for a storage facility at the box location", At 01:14 Ladder 18 reported smoke showing from the World Seafood Processing Warehouse located at 400B Dorchester Avenue. Car 6 arrived and transmitted a second alarm and ordered a evacuation of the fire building, Members dropped big lines and began a exterior operation as fire extended to the Gorham Fire Equipment building located at 30 D Street. Members stretched a blitz fire into the alley on side four for exposure protection while the deputy chief ordered a third alarm assignment. About five minutes later the incident commander reported a massive fire in a 120x40 1&2 story corrugated metal warehouse building and had fire alarm rebroadcast the evacuation order followed by transmission of a fourth alarm, engine companies on the fourth were requested to drop feeders into the fireground for ladder pipe operations. Jakes utilized three ladder pipes along with two tower ladders and numerous exterior lines to knock down the heavy fire condition that took hold of the building within two hours leaving an extensive overhaul operation that lasted into the daylight hours. Other crews arrived and drafted from a nearby pond and began tanker shuttle operations to maintain a water supply as the building began to collapse sending brands into the brush along the property and threatening neighboring buildings. Numerous spectators gathered in the area but were kept back at the Portside Road access gate and not allowed into the fire scene which was located further down on the point. Portsmouth was requested to respond with a brush piece and setup in the property behind the fire building for brand patrol, Members had the heavy volume of fire knocked down in about three hours but remained on scene well into the day chasing pockets of fire and investigating it's cause. At one point in the fire a large propane tank which fed the home began venting and subsequently caught fire and firefighters were evacuated from the area due to a potential bleve situation, Several area photographers found themselves in the backyard trapped and were forced to find an alternative way from the yard via a locked gate luckily no injuries were reported during the entire event. Members returned two days later to investigate a smoke condtition, Engine three arrived and ran a line for wetdown.

Engine 15 arrived on scene and reported a "code red" with fire showing from the basement on side four of the three story occupied woodframed building and stretched a line for fire suppression as Engine 14 arrived and dropped double feeders, Engine 3 (safety) Ladder 4 (fast) Engine 7 / Air Supply & Rescue 6 filled in on the working fire assignment. On scene companies made quick work of the overlapping fire using three lines which had the majority of the fire knocked down within ten minutes with minimal if any extension to floor one. The recall was sounded at 00:26 with no injuries were reported during the fire, the cause is under investigation.

Shortly before midnight uptown companies were dispatched to assist the police with a vehicle into a pole in the area of North Washington Street (Route 1) & Elmwood Street, A second report was given by the police reporting the vehicle on fire inside a dwelling and Car 3 requested the balance of a Level A response. Engine 5 arrived and reported a pickup truck into side four of a two story dwelling with fire extending from the vehicle into the basement area subsequently a working fire was transmitted which brought in Plainville Engine 3 to the scene and North Cumberland for station coverage. Companies on scene stretched a line and began to make entry via side one but were unable to make the basement due to the truck blocking the interior stairwell and crews were backed out and stretched in via the side two bulkhead. The second due engine was delayed in dropping supply lines as the hydrant that was once stood in front of the house was now wedged into the rear of the pickup truck Plainville arrived and assisted in dropping LDH to a hydrant on Summer Street. Members made quick work of the fire knocking it down before it could extend to the upper floor and attic of the structure, The electric company was requested due to a pole snapped and wires down which caused a power outage north of the incident. Battalion 2 reported heavy smoke from a distance as Rescue 1 arrived at 42 Arch Street and reported a heavily involved three story vacant wood frame, Engine 14 (safety) Ladder 6 (fast) Engine 7 / Air Supply & Rescue 4 responded on the working fire assignment. At 19:14 Battalion 2 arrived and transmitted a second alarm due to fire conditions and a potential exposure problem on side four, Engine's 10, 6 Ladder 5 & Division 1 responded on the second alarm as special hazards cleared a gas leak and took in the job. Incident command ordered a emergency evacuation of the building as heavy fire took hold of the upper floors and cockloft, Engine's 6 & 10 along with special called ladder 8 responded to Wilson Street to gain access to the rear of the fire building and setup for a exterior operation while fire alarm relocated Engine 2 & Ladder 7 to the Atwells Avenue firehouse. The officer of Engine 81 out of the College Drive station reported "smoke showing" on arrival from the China inn restaurant and setup command as the crew of second due Engine 83 out of the Burkholder Boulevard station arrived and dropped their own supply line and ran a mattydale to the interior for fire suppression, Tower 82 arrived and setup the bucket and ground ladders for egress and began to vent the roof. Upon further investigation interior crews uncovered fire in the vent system in the kitchen area with extension to the void space between the drop ceiling and roof decking and the crew of Engine 81 stretched a second line off 83 via a side two ground ladder and assisted members from the tower on the roof while additional firefighters stretched a third line into the building via side one. quint 99 stood by as the FAST truck, the fire was knocked down within a half hour with extensive damage to the kitchen area. Members from the Messier Street firehouse three blocks away were greeted with a smoke condition as companies responded and arrived on scene with fire in a large 3.5 story residential over commercial and began making entry via the rear. Battalion 2 arrived and special called an additional 1&1 over the working fire which consisted of Engine 11 (safety) Ladder 5 (fast) Engine 5/Air Supply & Rescue's 2&6 Engine 6 and Ladder 1 were dispatched as the special call. Heavy fire on floor one extended into the second floor which was split up into multiple apartments and an additional Engine and Ladder was special called and staged at the Messier Street station Engine 10 & ladder 4 were dispatched and later put to work while Engine 2 and Ladder 7 relocated to the Broad Street firehouse. Members stretched multiple lines and started to make progress about a half hour in with a extensive overhaul operation remaining. Memorial Day Weekend for Providence Jakes was unusually busy starting on Friday May 22 around 16:00 when firefighters were called to 333 Bucklin Street for a fire in a 1&2 story brick "L" shaped commercial building, Crews opened up and extinguished a small fire possibly caused by workers underneath the roof decking in less than twenty minutes.

Around 12:30 on Sunday morning Engine 13 responded to Terminal Road & New York Avenue on an investigation, On arrival the officer of the thirteens reported multiple fires burning in a large coal pile. Engine's 8 & 9, Ladder's 5 & 8 along with Division 1 responded to assist the unusual running order was due to the activation of Marine 1 which required specially trained members. Marine 1 utilized the deck gun for the knock down, Engine 13 was also working a fire in a scrap pile earlier on Saturday afternoon in the port which was knocked down with the trucks turret. At 13:26 Sunday afternoon Engine's 14,3,7 Special Hazard, Ladder's 6&1 with Battalion 2 arrived in the rear of Eastern Display located at 498 Kinsley Avenue to find multiple pallets and a fifty three foot trailer fully involved with fire extending to the roof line of a large 100x200 three story mill type building and a second alarm was sounded bringing in Engine 15,8 Ladder 4 & Division 1 along with the fill in assignment Engine 2, Tower Ladder 2, Engine 5/Air Supply as Heavy smoke could be seen for miles. Companies extinguished the main body of fire quickly with handlines and lines off Tower 1, It went under control in about a half hour. Later that afternoon around 16:25 Engine 14 was dispatched on an investigation of "Rubbish Burning In The Rear" on arrival at 37-39 Jones Street a large outside fire was discovered burning with fire extending to the exterior of a large vacant three story multiple dwelling the fire was quickly brought under control with Engine's 14,7,8,5,2 (Safety) Air Supply Special Hazards, Tower 1, Ladder 6, Ladder 7 (Fast) Rescue 6 & Division 1.Things remained quiet until about 01:50 when calls were received for a motor vehicle accident on Route 95 Southbound at Exit 20, Engine 7 arrived with a vehicle rollover with three patients injured one of which was ejected onto the roadway, All three patients were transported to Rhode Island Hospital for further treatment.

On Monday around 16:25 fire alarm dispatched Engine 3 to 21 Jones Street for a rubbish fire, Special Hazard reported "Smoke In That Vicinity and the fill in assignment was sent out with Engine's 3,14,7 Special Hazard, Tower 1, Ladder 4 & Division 1 responding. Hazards arrived with a three story woodframe that had been demolished well involved Jakes made quick work with handlines and elevated lines from Tower 1 which setup on Cedar Street crews had knocked down the fire in about fifteen minutes. Around 19:40 the vocals opened up again "Engine's 9,7,5....228 Transit Street for a Stove Fire" As the scanner went back into scan mode Engine 9 arrived on scene and reported a "Code Red" with fire in the first floor kitchen area of a 2.5 story occupied woodframe, This fire was also quickly knocked down and contained. Members took a much needed rest until the tones went off at 23:40 for a possible fire in the wall at 152 Gallup Street, Engine 10 arrived first due with nothing showing, Upon further investigation fire was discovered in the third floor wall of the three story occupied dwelling and was knocked down and contained to one bay with one line without extension.

Shortly after Tuesday started once again the tones went off "Attention Engine Companies 12,2,7 Special Hazard, Ladder's 3,7 & Battalion 3 a still box...284 Admiral Street reported house fire". It didn't take long for Engine 12 to come across channel two with a code red as the company arrived with fire showing from a vacant 2.5 story woodframe. An aggressive interior attack ensued which knocked down the heavy volume of fire in about twenty minutes after arrival leaving only an extensive overhaul operation.

Shortly after 11:30 Monday night Smithfield fire alarm received numerous calls for a structural fire at 99 Pleasant View Avenue. As all town fire companies responded Police arrived and confirmed a fire in the building, Shortly thereafter Engine 1 arrived with heavy fire showing from the one story strip mall. On arrival of the chief an additional 1&1 was requested over the working fire assignment bringing in Johnston Ladder 1 and Glocester (Harmony) Engine 21 for the RIT as heavy fire broke thru the roof. Command ordered a defensive operation to to building construction and fire conditions multiple handlines made quick work of the fire which was knocked down and placed under control in about an hour with no injuries reported. Pratt Dam Water Rescue In Cumberland

On Friday afternoon Cumberland firefighters were dispatched to the Blackstone River behind the stop & shop supermarket at 70 Mendon Road for a report of two canoeist in the water. Members from Engine 22 happened to be at the supermarket and quickly arrived at the Pratt Dam with two people in the river. Mutual aid from Lincoln responded along with the activation of the Blackstone Valley Dive Team which brought in additional crews out of Woonsocket as members went to work on extricating the first patient who was closer to shore. Firefighters then went to work on extricating the second patient who was pinned up against the dam due to water currents, members used ropes and an inflatable raft to approach and remove the patient who was then evaluated by rescue, the entire incident lasted about an hour. Today's rescue is in the same location of a similar incident which claimed the life of a 17 year old boy who was canoeing thru the "tubes" near the dam with his father, the vessel overturned and became hung up on trees and other debris in the water trapping the boy under water his father was safely removed that incident occurred on Sunday, November 9, 2008.

At 03:55 channel two opened up "Engine 11 calling fire alarm, Engine 11 has a Code Red" Battalion 2 subsequently requested for an additional 1&1 as the elevens reported heavy fire venting from side one. Engine 8 & Tower Ladder 1 (Special Call) along with Engine 3 (Safety) Ladder 6 (Fast) Engine 5 & Air Supply (Fill In Assignment) responded to the alarm as Battalion 2 arrived and set up command. Heavy fire took hold of the first and second floors of the 20x30 2.5 story occupied woodframe dwelling located at 1239 Elmwood Avenue on the Cranston city line, at the ten minute mark of the aggressive interior attack companies had five lines run and were making good progress. Late On Thursday Afternoon Pawtucket Engine 4 & Rescue 2 were dispatched to on John Street at Bates Street for a reported person down in the roadway with gun shot wounds. Engine 4 arrived with a person down on Coyle Avenue which is one block away shortly thereafter Rescue 2 arrived and began treatment, The victim was transported to Hasbro Childrens Hospital in Providence for further treatment as Police set up a crime scene in front of a three decker just in from the corner. Crime Scenes were also set up further down the street towards Mendon Avenue and On Coyle Street where the patient was found, The shooter is believed to be in custody. The area where the shooting occurred has been plagued with violence in the past including a police involved shooting In July Of 2007.

Engine 13 arrived minutes later with a vehicle up against a three story row house and began to access injuries to the driver seconds later Engine thirteen's portable opened up, "Engine 13 to fire alarm urgent! we have a explosion at this location" fire alarm responded "received we will send out a full still box, Engine 13" as what was left to the one story portion of the structure littered the street and immediate area. Members from the Police Department flooded into the area as the fifty or so spectators ran in the opposite direction, Engine 10 arrived and stretched a line to extinguish a fire which was burning under the collapsed section of the building while Ladder 5 arrived and went to the roof. A half dozen members were transported to Rhode Island Hospital with unknown injuries along with a police officer. National Grid & The Building Inspector were requested to the scene as detectives setup a crime scene. The American red cross & Members from the special signal fire association also were on scene. The cause of the explosion is still under investigation but is likely caused by the vehicle striking the exterior gas main to the apartment complex.

You have all seen the trucks at scenes such as the Uxbridge Mill Fire, Worcester cold Storage Warehouse Fire, Missing Person Searches, Police Incidents and most notably the truck responded on September 11th. After a meeting today, The Salvation Army decided to close the doors to the canteen station in the old Pawtuxet Village firehouse located at 34 Commercial Street in Cranston. Major Burr decided to run the two trucks with Salvation Army Personnel only to third alarms or better leaving the members from the Special Signal who have ran the trucks for seventeen years out on our own, Burr has also decided to sell the waterfront property which he acquired for a dollar from the special signal fire association and also had the locks changed an hour after Tuesdays meeting. The members from the special signal are working diligently to acquire a new truck which is in the process of being manufactured, Members of Services should not see a lack of rehab services as we are responding in our personal vehicles to the scenes.

Around 11:30 Friday Night Lincoln firefighters were dispatched to 47 Old River Road for a heater fire in a occupied dwelling, Police arrived on scene and reported fire showing from the rear of the one story woodframe and reported a patient with minor burns subsequently rescue 2 was dispatched shortly thereafter radio traffic broke thru "Chief 7 is on scene with a code red!" and the F.A.S.T Truck was dispatched out of Albion. Quinnville Engine 76 arrived first due and ran a line to initiate fire suppression, while Members from Lonsdale Engine 4, Albion Ladder 20, Lime Rock Engine 31 & Saylesville Ladder 51 arrived and assisted in the aggressive interior attack. Heavy fire took hold of the dwelling impinging on a propane tank which was left on the rear deck, members quickly used a handline to cool down the venting tank and remove it to prevent a boiling liquid expanding vapor explosion which they did without incident. Members trying to gain entry via the rear had some tense moments as a members foot went thru the floor due to holes at the doorway which caused unsafe conditions and a egress issue to the interior companies who entered via side one, the jake was safely removed and continued working. Members got a handle on it about a half hour later using three handlines, leaving only a extensive overhaul operation that lasted into the early hours of Saturday morning. One occupant was evaluated on scene by members of Rescue 2 and refused further medical treatment against medical advice, the cause of the fire is still under investigation by Lincoln Police & the state fire marshal, The Providence Canteen responded to provide rehab for the members.

Two Alarm Commercial Building Fire In Attleboro

Shorty before ten Sunday Night Attleboro firefighters responded to a box alarm for the General Metal Finishing Company located 42 Frank Mossberg Drive in the South Attleboro section of town, Engine 2 arrived first due and reported heavy smoke showing from the large one story 50x200 commercial building and setup for ladder pipe operations with Ladder 2. The General Metal Finishing Company has been in operation since 1972, electroplating metal products for the medical and automotive industries just to name a few and employing over a hundred people. Due to multiple different types of hazardous materials stored in the structure and a heavy fire condition which self venting thru the roof in the rear a defensive operation was utilized with elevated pipes from both ladder trucks and multiple handlines, A Pawtucket Engine was also special called due to a water supply problem in the industrial park and arrived and dropped lines into the sprinkler system as firefighters started to gain the upper hand on the fire. All firefighters working in the area were "on air" due to levels of cyanide which were detected, members had the heavy volume of fire knocked down in about an hour with an extensive overhaul operation that lasted into pre dawn hours of Monday, no injuries were reported the Providence Canteen responded along with numerous Providence Citywide members. The cause is under investigation, the rear of the building was heavily damaged but the fire was held to that area without further extension thanks to the sprinkler system.

Around 05:30 Thursday Morning firefighters from the Portsmouth Fire Department responded to a report of "Smoke In The Building" at New England Boatworks building 9 located at 9 Lagoon Road. First arriving companies reported "smoke showing" moments later fire was venting thru the roof of the 50x200 one story Quonset hut which sat in the middle of a row of seven similar type buildings. A third alarm was transmitted quickly which brought in mutual aid to the fire and to cover the station, due to a heavy fire condition firefighters on the fireground utilized a defensive operation as fire extended to exposures on sides two and four (using water as side 1) and a fourth alarm was struck. Middletown Ladder 39, Navy Ladder 1 & Portsmouth Tower 1 used ladder pipes to gain the upper hand while additional members drafted from Narraganset Bay to maintain a water supply due to a lack of water pressure on yard hydrants. Incident command requested Providence Marine 1, Cranston Marine 2, & East Providence Marine 3 to the scene along with the East Providence & Newport hazardous materials response teams due to unknown chemicals which were exposed during the fire DEM & The US Coast Guard were also notified. Firefighters From Portsmouth, Navy, Tiverton, Bristol, Swansea, Middletown, Newport, & Warren worked in a surround & drown operation for hours before the heavy fire subsided while Fall River Covered, Due to high levels of cyanide several members were transported to area hospitals for further evaluation, All firefighters were also required to go thru decon prior to leaving the scene. Excavation equipment was called in to assist with exposing hotspots and help with access to investigators later that afternoon. Engine's 3,14 Tower ladder 1 & Rescue 2 (Special Call) along with Engine 15 (safety) Ladder 6 (fast) Engine 5 & Air Supply (fill in assignment) responded as members on the fireground began to make some headway on the overlapping fire. Firefighters made an aggressive interior attack to preform primary & secondary searches and extinguish the remaining fire which was auto extending out side three making the rear awning structurally unstable, firefighters also had to deal with a live electrical wire down onto a metal fence which made it an electrocution hazard. The Gorham Manufacturing Company operated at 333 Adelade Avenue in the Elmwood Section of the city from 1890 till 1985, the complex was made up of twenty-one brick one to three story buildings in a area of thirty seven acres bordered to the North and West by Mashapaug Pond and the East by train tracks. The company manufactured precious metals and had one of the worlds largest foundries operating at the site. After the company relocated to Smithfield in 1985 the buildings sat vacant becoming a nuisance with frequent fires one of which leveled a main portion of the complex in 1992, By 1995 many of the buildings were gone sparing the carriage house which was proposed to be the site of the Providence Fire Museum in 1999 A Stop & Shop Supermarket was also opened on the site that same year and has since become vacant.

Providence firefighters retired and active began working to rehab the structure in July of 2002 with substantial progress being made. In early 2009 renovations looked to be headed in the right direction with new masonry work done to the exterior and a new roof installed, the building was often frequented by vandals and graffiti artists due to its location behind the strip mall.

On Wednesday Afternoon around 4:15 all hopes for a fire museum in the historic building were lost as a long tone opened up followed by "Attention Engine Companies 11,10 & 8 Special Hazards, Ladder 5, Tower Ladder 2 & Battalion 2 a Still Box, to the rear of Walgreens, 533 Elmwood Avenue reported building fire." A large loom was visible to firefighters leaving the Reservoir Avenue station, moments later Engine 11 arrived on scene reporting the Code Red. While fireground operations on channel two, "Lay A Line Into Ladder 5!" the fill in assignment went out on channel one, "Engine 14 (safety) Ladder 6 (fast) Engine 5/Air Supply fill in on a still box" firefighters on scene could only watch as the building they had invested so much time into was consumed in a heavy fire condition which set off spot fires along the pond, Engine 5 was assigned as exposure sector and handled those fires. The large vacant two story "L" shaped brick & wood carriage house was the last building left from the original Gorham Company and a part of Rhode Island History it was a total loss!

At 08:28 on a rainy Sunday Morning a long tone followed by "Attention Engine companies 14,15,12 Special Hazards Ladder's 6 & 3 and Battalion 3 a still box" was all that was needed to be said for all hopes for a quite morning in the capitol city to be shattered as a reported occupied house fire at 52 Zone Street was called in, due to multiple calls the working fire box was transmitted prior to arrival Engine 3 (Safety) Tower Ladder 1 (Fast) Engine 5/Air Supply & Rescue 6 filled in on the assignment. Engine 14 reported a "smoke condition" while enroute and at 08:30 arrived on scene of a 2.5 story woodframe with floor two well involved extending to exposure four which is a similar type structure and a working fire was confirmed the officer of the fourteens requested an additional engine and truck (Engine 7,Ladder 4) on arrival while members stretched a line to play in between the two buildings. Due to the exposure problems on sides two & four a second alarm was automatically struck on the arrival of Battalion 3, at 08:38 Engine's 2,8 Ladder 7, Rescue 5 & Division 1 responded on the alarm followed by Engine 9 who was special called for a safety crew after the initial safety company was put to work with exposure protection. Fire eventually extended into the half story and dropped down into floor one before being knocked down in typical Providence fashion in less than a half hour. One person was trapped on the roof and was rescued prior to arrival by bystanders Rescue 4 was special called to transport the patient for evaluation followed by an additional special called for a relief company with Engine 11 responding. Firefighters made great progress holding the heavy fire conditions to the building of origin located at number 52 Exposure two at number 46 sustained melted siding due to the radiant heat, Exposure four located at 54-56 received heavy charring to the exterior on floor two and interior water damage the fire didn't extend to the interior of that building the recall was sounded at 10:40.

Around 03:00 Thursday morning Middleborough firefighters were dispatched to 42 Vernon Street for a building fire, First arriving companies had heavy fire showing from a large 2.5 story occupied woodframed dwelling and companies went to work. Due to a lack of hydrants in the area numerous mutual aid tankers from surrounding Plymouth & Bristol Counties were special called over the second alarm assignment which was requested on arrival. Firefighters utilized an surround & drown operation along with tanker shuttle operations for hours with two tower ladders and multiple large lines in use which finally darkened down the heavy volume of fire shortly before sunrise. Due to a major structural collapse of the fire building heavy machinery was called in to assist with overhaul. The Providence Canteen and Massachusetts State Fire Services Rehab Unit were also requested to the scene no injuries were reported during the fire the cause is still under investigation.

Around 02:00 hours on Saturday morning Providence fire alarm dispatched Engine 11 & two rescues to Elmwood Avenue & Roger Williams Avenue for a motor vehicle accident with several injured. While crews responded multiple calls began coming in for the wreck reporting two ejected and occupants trapped inside and under the vehicle and the balance of a technical rescue response was dispatched (Ladder 5, Special Hazards & Battalion 2). Engine 11 arrived with a two car accident with multiple injuries and a third and fourth rescues were requested, Special hazards arrived and setup for extrication using the hurst tool to remove one occupant from the sports utility vehicle while additional members tended to the injured. Once extrication was complete the casualties were transported to Rhode Island Hospital with two in serious condition (unconscious) and several who were "walking wounded" on scene and transported for evaluation. Providence BCI & Detectives were also requested to investigate the incident.

Shortly after 20:00 hours on Friday the 13th Engine's 5,1,2 Tower 1 & Battalion Chief responded to the area of Baldwin & Cooper Street which actually don't intersect for a reported basement fire, Due to multiple calls and police on scene reporting a active fire at 71-73 Baldwin Street the fill in assignment Engine 3, Ladder 2 & Rescue 1 were dispatched. Engine 5 arrived first due with a three story vacant flattop woodframed building with fire in the basement on side two extending into the first floor and a line was stretched for fire suppression. A second and third alarm were struck as heavy fire in the basement rapidly extending into the upper floors and threatened exposures on sides two and four. Members from Group C made an aggressive interior attack but were forced to evacuate due to multiple holes in the floors and progression of fire, Crews utilized a deck gun, ladder pipe and multiple large lines from the exterior to gain control of the fire in about a half hour with an extensive overhaul operation remaining into the evening. No serious injuries were reported during the fire, one firefighter put his leg thru the floor making entry and was safely removed another great stop by Pawtucket jakes!

Around 21:30 Sunday Night Providence firefighters responded to Hyat & Kossuth Streets for multiple calls reporting a building fire. Engine 15 the first due company transmitted a second alarm with a three story vacant woodframe well involved and a second three story dwelling on side two with heavy fire showing from the first & second floors. An interior attack was initiated on the exposure building while incident command arrived and reported a defensive operation on the building of origin, multiple exposures which were all vacant dwelling were also being threatened and third alarm shortly followed. Heavy smoke could be seen for miles as Providence jakes knocked down the fire in the two buildings while additional companies utilized deck guns and ladder pipes to extinguish the fire in the building of origin. Once the heavy fire was knocked down and contained in the exposure building lines were used for containment of the first building. Two firefighters were transported to a area hospital with minor injuries suffered in a collapse of the rear stairwell on side four. The cause is still under investigation but is most likely arson due to the amount of fire on arrival and the buildings being vacant.

Shortly after 20:00 hours on Saturday Pawtucket Fire alarm began receiving numerous reports of a structural fire at 160 Meadow Street Engine's 3,6,4 Ladder 2 & the Battalion Chief responded due to the amount of calls and reported entrapment Engine 2, Tower 1 & Rescue 2 were sent out prior to arrival. Engine 3 arrived first due, the officer transmitted a second alarm with numerous people trapped on the second floor and heavy fire showing from the entire first floor. Ground ladders were thrown to the occupants on side one which allowed them to safely get to the ground. Due to a occupant still trapped on floor three a aggressive interior operation was initiated running lines into the heavily involved first floor which by now was extending to the upper floors and also running lines over ground ladders due to fire load in the stairwells. Engine 4 made the third floor and located a male victim who was removed via side two and transported to Memorial Hospital where he later died. Firefighters made great progress knocking down the overlapping fire which was impinging on the exposure two building which was of similar type in less than an hour leaving an extensive overhaul operation. One firefighter received a minor hand injury and was transported. All city companies responded to the alarm which required mutual aid to respond into the city for coverage, incident command special called a Central Falls Engine from the cover assignment for the R.I.T or F.A.S.T Company and a Central Falls Rescue to standby. The State fire marshal & Fire Prevention were on scene investigating the cause which is being called accidental.

The fire was located less than a block from the site of a fatal fire that occurred on Thursday, February, 2 2006 at 177 Meadow Street. A 21 Year old set fire to the home and a vehicle which was parked in the driveway. The 95 year old resident was trapped in his bedroom he was removed and transported to Memorial Hospital then later transferred to Rhode Island Hospital where he remained on life support for five days before he died.The 21 year old who started the fire was found in Ohio and extradited back to Rhode Island where he faced murder and arson charges.

At 18:49 Pawtucket Fire Alarm dispatched Engine's 6,3,4 Ladder 2 & Battalion 3 to 14 Liberty Street for a reported basement fire, Engine's 3 & 6 arrived simultaneously and reported a smoke condition. The officer of Engine 3 reported a "Code Red For Responding Apparatus" and requested a line to the basement, Upon further investigation fire was discovered extending upwards into the first floor and a second line was stretched followed by a request for the fill in companies Engine 2, Tower Ladder 1 & Rescue 2 were dispatched with Ladder 1 approaching off Gilmore Street. Rescue 1 was special called to transport a patient with burns to Memorial Hospital as fire was uncovered traveling up via the chimney into floor two and the attic area and a third line was stretched. Engine 3 arrived and reported fire showing from the second floor side two of the previous fire building and stretched one line to extinguish a mattress and other contents within the room, The fill in companies were staged on scene while Ladder 2 was requested to overhaul and remove smoldering debris to ensure that they wouldn't be back later in the evening. No injuries were reported during the second incident.

Around 04:00 Monday Morning Pawtucket Fire Alarm Dispatched Engine's 3,6,4 Ladder 2 and The Battalion Chief To 170 York Avenue for a reported structure fire. While crews responded fire alarm notified the chief of multiple calls being received thru 9-1-1 and the fill in assignment Engine 2, Tower 1 and Rescue 2 were sent out. Lieutenant Carroll reported a fully involved mill building as Engine 3 was approaching the scene and transmitted a second alarm on arrival, Mutual aid Box 2826 was struck for station coverage at that point. Incident command arrived and ordered fireground operations to remain defensive while fire consumed the one story gym type structure and threatened the exposure four building which is a two story metal clad commercial building with a tin roof. Ladder 2 arrived and setup a ladder pipe on side one while Tower 1 setup on side four and setup to flow water, Engine's 3 & 2 also played water onto the fire via deckguns. Car 4 (Safety) reported heavy fire extension in one of the commercial establishments in the exposure building and requested manpower to side four, Providence Ladder 4 who was on the cover assignment was special called and setup a additional ladder pipe on side one. Members started opening up the businesses on side four and uncovered several which were well involved with vehicles also involved and additional handlines were stretched for extinguishment as the wall on side two collapsed in. Firefighters made great progress knocking down the heavy volume fire in about two hours in the extreme weather conditions, Incident command requested callbacks to man reserve apparatus due to the extended overhaul operation which lasted well into the day.

The exposure building was the scene of a two alarm fire on Tuesday, October 7, 2008 and had since reopened for business, Pawtucket Fire Prevention and The State Fire Marshal is still investigating the cause but have the owner of the gym in custody and have charged him with arson, No injuries were reported during the incident.

Engine 3 cleared the scene at 10:28 and was dispatched back to investigate smoke from the building shortly before 19:00 the report was unfounded. Engine 3 & Engine 6 were dispatched back again at 23:48 for a small fire on side four called in by the police detail.
